If you know you're planning on attending Driven Church for the first time with your children, an easy way to save time on Sunday morning is to pre-register your kids for our children's ministry programs using the form below.

NOTE: Please fill out a new form for each of your children. For example, if you have three children, please submit three individual forms; one for each child.
